There are several ways to obtain a VO₂ max value, from respiratory-gas testing to an equation with no exercise at all. The best choice depends on why you want the number, whether exertion is appropriate, and how much uncertainty you can accept.

Laboratory cardiopulmonary exercise testing

During a cardiopulmonary exercise test, workload rises while equipment measures inhaled and exhaled gases. The test can directly characterize peak oxygen use under its protocol. It also has the practical advantages and constraints of specialist equipment, trained supervision, effort criteria, and a specific treadmill or cycle mode.

The 2022 FRIEND reference study demonstrates why mode matters: the published treadmill and cycle reference values differed. Fitness Age Calculator’s current direct-measurement route therefore accepts only an eligible maximal treadmill result with clear provenance.

One-mile walk estimate

The Rockport equation estimates VO₂ max after a fast, measured one-mile walk. The original study included healthy adults aged 30–69 and used body mass, age, sex coefficient, walk time, and immediate finish heart rate. It is accessible but depends on a safe, exact course and accurate timing and pulse capture.

Non-exercise estimate

When a physical test is not appropriate or convenient, the Quick Assessment uses inputs such as resting heart rate, body size, and a defined activity category. The Jurca et al. research supports this low-friction approach at a population level, while the reported standard error makes clear why individual confidence stays low.

Wearable estimate

Smartwatches can make repeated estimates from exercise or resting data. In a systematic review and meta-analysis, exercise-based wearable algorithms showed lower average error than resting-information algorithms, but the individual limits of agreement were still broad. Algorithms and firmware can also change.

Choosing and tracking a method

Use a laboratory measurement when high-quality measurement is important and an appropriate service is available. Use a field test when its sample, protocol, and safety limits fit. Use a non-exercise method for a quick starting point. Treat a wearable as its own ongoing series rather than a drop-in laboratory value.

Always save the unit, date, method, mode, conditions, and source. Compare repeat results made with the same method. If values from different methods disagree, do not average them; investigate their protocols and uncertainty.
