A smartwatch and a laboratory can display the same VO₂ max unit while arriving there differently. A watch estimates from sensor data and an algorithm. A laboratory cardiopulmonary exercise test measures respiratory gases during a controlled, progressively harder test.
That difference explains why two values may not match—and why neither should be copied into a reference built for the other without checking the method.
What a watch is estimating
Consumer wearables may combine speed, heart rate, movement, age, body information, and recent training data. The exact inputs and model can vary by device and software version. Poor sensor contact, hills, wind, heat, pauses, wrist motion, medication, and an inaccurate maximum-heart-rate setting can all influence the estimate.
A 2022 systematic review and meta-analysis found that exercise-based wearable algorithms had little average bias across pooled studies, but their individual limits of agreement remained wide. Resting-information algorithms showed larger average overestimation. A small average group error does not mean every individual estimate is close.
What the laboratory adds
Respiratory-gas analysis directly tracks oxygen and carbon-dioxide exchange while workload rises. Test staff can document treadmill or cycle mode, effort criteria, equipment, and why the test ended. The result still has measurement and protocol variation; “lab” is not a magic guarantee of identical numbers everywhere.
The updated FRIEND study reported different reference values for treadmill and cycle cardiopulmonary exercise tests. Even two direct measurements can require different interpretation when the mode changes.
Which result should you use?
For clinical questions, supervised testing decisions belong with a qualified professional. For high-stakes sports testing, use a method and protocol suited to that purpose. For everyday trends, a watch can be convenient if you treat it as a consistent device-specific series.
Do not average the watch and lab numbers. Save each with its date, source, mode, conditions, and software or protocol when known. Expect a new watch, firmware update, activity type, or maximum-heart-rate setting to create a break in the series.
